Health Forms

In February, parents of new and returning ANC students should have received a package of information from the ANC Admissions Office that included a Health Form for the 2011–2012 academic year. This single bi-fold form contains all the information and consent forms that parents are required to return to the Doering Health Clinic.

No student will be permitted to come on campus or participate in any activity, including entering the dormitory, participating in sports, or attending any orientation events unless this form is completed and returned by June 30. Incomplete records will delay a student beginning their school year, so parents must be sure to meet vaccine requirements as noted on the form.

Training Contract

As part of our effort to actively promote our mission and values, the ANC Athletic Department has established Training Rules [pdf] to support our athletes in maintaining a healthy and moral lifestyle.
