Student Health Care
Services
Student Health Service is located on the Academy campus in the lower level of the Dining Hall. Student Health offers a variety of services including morning clinic, health screenings, health teaching, and management of minor illness and sports injuries.
Dormitory students receive assistance with medication management, appointments for health care and obtaining health services needed during the school year.
Student Health also monitors immunization status and school health records in accordance with State of Pennsylvania requirements.
Personnel
The clinic is headed by Allyn Simons, RN, MSN, CSN (full time) assisted by Jenny Blair, RN. The consulting M.D. is Dr. Bruce S. Morrison (part time) whose practice is located about two miles from the ANC campus.
The nurse is on campus Monday through Friday during the school day. After school hours and on weekends a nurse is available on-call at all times.
Health Forms
What about all those forms? Here is a Health Forms 101 to help you sort them all out:
Emergency trip form (pdf): Completed by parents. Gives permission for chaperones to take student to hospital or seek medical care. Gives insurance information and a signature giving permission to treat. Does not have any physical information included.
Complete physical: Completed by Doctor. Must be submitted when the student first enters school (9th) and again in Junior year. This is a more lengthy form that INCLUDES the exam items necessary for the sports participation physical. Voila'! Two physicals in one.
Sports physical (pdf): Completed by Doctor. An abbreviated physical assessment necessary for the student to participate in sports. This is required new every year when students wish to participate in sports, and the abbreviated version is for returning students and non-Juniors who do not need to provide the complete physical.
