1. Registration

      To hold your spot, print out the registration form and mail it with a check for the correct amount to:

2. Health Forms 

    Print and Fill Out the Applicable Forms: 

•     Basic Health Form if registrant is 18 years or younger

•     Standing Orders for Medication Form if registrant is 18 years or younger

•     Permission and Release Form if registrant is 18 years or younger and staying on campus

3. Housing

    If you are requesting on campus housing, please contact Summer Institutes Director Erik Odhner.
